Doctors, insurance and emergencies in Stockholm
Region Stockholm charges one flat fee for almost every outpatient visit, with a ceiling that runs on a rolling year. Missing an appointment costs more than attending it.
The thing people get wrongThe high-cost ceiling runs over twelve months counted from your first paid visit, not over the calendar year, so keep track from the first receipt. Cancel appointments more than 24 hours ahead, because a no-show costs SEK 400, more than the visit itself.
One fee, whichever door
Region Stockholm charges SEK 275 for an outpatient visit, and it is the same SEK 275 at a vårdcentral, a specialist clinic, a hearing clinic or a digital appointment. A video consultation therefore saves you time rather than money. The schedule only applies to care the region has contracted providers to deliver, which is why a purely private clinic's bill bears no relation to it. Telephone advice and calls to 1177 are free.
Urgent care, and the cheaper door
A närakut, an urgent care centre, costs SEK 375, and a hospital emergency department costs SEK 400, so the närakut saves money as well as hours for anything that is not an emergency. 112 is for life-threatening situations and nothing else. 1177 is the national health advice line, staffed around the clock, and calling it before you choose a door is the cheapest decision in the system.
The ceiling and the frikort
The high-cost ceiling for outpatient care, högkostnadsskydd, is SEK 1,450, counted over a rolling twelve months from your first paid visit rather than over the calendar year. Once you reach it you are issued a frikort that covers further outpatient visits for the rest of that period, and it is valid in every Swedish region rather than only in Stockholm. Children and young people under 18 pay nothing at all, and adults of 85 and over pay nothing for outpatient care within the cost-protection system. Missing an appointment without cancelling at least 24 hours in advance costs SEK 400, and that charge does not count towards anything.
Listing with a vårdcentral
You list yourself with a vårdcentral, normally by logging in at 1177.se, and you can change that listing at most twice in a year unless there are special circumstances. You do not have to wait for the listing to be treated, though: you can seek care at a vårdcentral when you are ill even if you are not listed there, and you may seek care anywhere in the country, although a referral is sometimes required. That is the practical answer for the window before your registration is finished.
