Doctors, insurance and emergencies in Colombo
Good private hospitals in Colombo, a free public system and a free national ambulance — with private care paid up front unless your insurer direct-bills.
The thing people get wrongPrivate hospitals expect payment, or an insurer's guarantee, before treatment and admission. Check that your policy direct-bills the Colombo hospitals near you, or be ready to pay a large deposit at the desk and claim it back.

Private hospitals and channelling
Colombo's private hospitals — Asiri, Nawaloka, Lanka Hospitals, Durdans and Hemas among them — handle routine, specialist and emergency care with English-speaking doctors. Specialists are seen by 'channelling': booking a consultant's session at a hospital, usually by phone, the hospital's app or the eChannelling service. Consultation fees are modest by international standards; admissions and procedures are where the bills add up.
- Book specialists through the hospital or eChannelling rather than turning up.
- Ask for an estimate before any admission or procedure.
- Keep itemised bills and reports for your insurer.
The public system
Government hospitals, including the National Hospital of Sri Lanka in Colombo, provide care free at the point of use and see very large numbers of patients. The clinical staff are capable, but waits are long and conditions basic. Most foreign residents use private hospitals for anything they can plan and keep the public accident service for genuine emergencies.
- Free at the point of use, with long waits.
- The National Hospital's accident service handles major trauma.
- Private hospitals are the practical choice for planned care.
Insurance
Buy international health insurance before you arrive, and check that it direct-bills the private hospitals you would actually use. Most care can be done in Colombo, but some complex cases are referred to Singapore, India or Bangkok, so look at the evacuation cover and who decides when it is used.
- Direct billing with Colombo's main private hospitals.
- Medical evacuation, with the trigger and destination spelled out.
- Pre-existing conditions and waiting periods, in writing.
- The nomad and residence visas may ask for proof of cover.
Everyday health
Pharmacies are everywhere, including the state Osu Sala chain, and many medicines are sold over the counter. Tap water is not drunk untreated; boil or filter it. Dengue is present year-round and rises with the monsoon rains, so mosquito precautions matter. Road traffic is the main everyday risk, especially for pedestrians on Galle Road and other main roads.
- Use established pharmacies and keep prescriptions with you.
- Bottled, boiled or filtered water for drinking.
- Repellent and screens: dengue peaks with the rains.
- Cross main roads at signals; traffic rarely yields.
