Getting into the system

Employment normally brings public cover with it, so ask your employer to confirm the registration has been filed rather than assuming it. If nothing covers you, you can buy in by signing a contract with the regional NFZ branch for where you live, which in Warsaw is the Mazowiecki oddział. The premium is 9% of declared income with a statutory floor, and for the July to September 2026 quarter that floor is PLN 845.67 a month, calculated on an average wage of PLN 9,396.33. Non-EU students pay PLN 55.80 a month and volunteers PLN 432.54.

• Take an identity document, the application form in two copies, ZUS forms ZZA and ZCNA if you are also registering family members, evidence of your previous insurance history, and proof of the entry surcharge if one is due.
• Non-EU nationals also show proof of residence in Poland. The application can go in person, by post or by electronic registered mail.

The price of a gap

The voluntary contract carries a one-off entry surcharge scaled to how long you went uninsured. A gap of three to twelve months costs PLN 1,879.27, payable in full. One to two years is PLN 4,698.16, two to five years PLN 9,396.33, five to ten years PLN 14,094.50, and more than ten years PLN 18,792.66, each of which the NFZ branch director may spread over instalments in a justified case. Volunteers and certain foreign students are exempt.

Registering with a doctor

You attach yourself to a named doctor by filing a deklaracja wyboru, a written declaration of choice of doctor, nurse or midwife, either at the clinic or through the Internetowe Konto Pacjenta. Primary care then covers preventive care and vaccinations, diagnosis with laboratory and imaging tests such as ECG, X-ray and ultrasound, referrals to specialists, hospital, rehabilitation and sanatorium treatment, and home visits where they are medically justified. The patient account at pacjent.gov.pl and the mojeIKP app hold your prescriptions, referrals, sick notes and records, and handle central booking for screening programmes. Access runs through Profil Zaufany, so in practice it waits on your PESEL.

Hours, and what to do outside them

Public primary care runs Monday to Friday from 08:00 to 18:00, excluding public holidays. Outside those hours, nocna i świąteczna opieka zdrowotna covers 18:00 to 08:00 on working days and the whole of a holiday, with walk-in consultations, telephone advice and home visits where justified. You may use any night-and-holiday location whatever clinic you are registered with, which is useful in the weeks before you are registered anywhere. A hospital emergency department is for accidents, trauma, poisoning and internal bleeding, and it takes patients in order of medical severity rather than arrival, which is why a non-urgent visit there means a very long wait.

• Call 112 or 999 for a sudden threat to life or health: loss of consciousness, seizures, severe burns, vomiting blood, acute chest pain.
• The NFZ patient information line, 800 190 590, is free and staffed 24 hours a day.

Private care as a stopgap

Until you are inside the public system, private appointments are the practical answer and are booked directly. Prices listed for Warsaw in September 2026 put an internist, the private-sector equivalent of a GP, at PLN 200 to 350 and most often PLN 250 to 300, and a dermatologist at PLN 250 to 400. Those are what clinics advertise on a booking site rather than a tariff, so treat them as the going rate and confirm with the individual clinic.
