Registering at your centro de saúde

You register in person at the health centre for your area of residence, and you do not need to obtain the número nacional de utente in advance, because it is issued at the moment you register. Bring your residence permit, for its number and its validity, your civil identification number, your NIF, proof of the permanent Portuguese address you live at, and evidence of any exemption you are claiming. Registration can be done at any SNS primary-care or hospital unit, and a number is also generated when you apply for a Cartão de Cidadão. If you do not know which health centre covers your address, SNS 24 on 808 24 24 24 will tell you.

• Where you have no residence authorisation document, a rental contract or a utility bill is accepted as the proof of address.
• EU citizens and non-EU nationals holding a valid residence authorisation get a full registration in the Registo Nacional de Utentes and full SNS access, with the cost of care borne by the SNS.

Before your residence permit is issued

You are not shut out, but you are limited. Registration stays in progress and you pay for care, with exceptions for maternal and child health, reproductive health, care of minors, vaccination and proven economic hardship. Urgent and vital care is free on production of a declaration from your junta de freguesia proving more than 90 days' residence in Portugal, which is worth obtaining before anyone in the family needs it. Children under 18 need neither a residence permit nor a NIF for a full registration and get a further 90-day grace period, so a family can register its children before the adults' paperwork is finished.

User fees, and where they still apply

Taxas moderadoras have largely been abolished and what survives bites in essentially one place: attending a hospital emergency department without a prior SNS referral. The visit is free if your health centre or SNS 24 referred you, or if you are admitted to hospital through the emergency department, and the framework law is Decreto-Lei n.º 113/2011, de 29 de novembro. The health regulator confirms the charge exists but publishes no amount and refers enquirers to ACSS, so ask the hospital or ACSS what an unreferred attendance costs rather than trusting a figure from a forum.

• Exempt categories include pregnant women, minors, people with 60% or greater disability, blood donors, transplant recipients, firefighters, military personnel and asylum seekers.
• Exemption for insufficient means in 2026 is household average monthly income per income-earner of no more than €805.70, which is 1.5 times the IAS of €537.13. It is indexed, so ask for the current figure.

Private care in the metropolitan area

The private groups with units around Porto include Trofa Saúde, at Maia, Gaia and Boa Nova in Matosinhos, and Lusíadas; Hospital da Luz and CUF also operate in Portugal. None of them publishes a consultation price list on its public website, and Trofa Saúde tells enquirers to telephone. Ask for the cash price and the price under whatever insurance you hold, and do not assume a figure quoted for Lisbon holds here.
