Doctors, insurance and emergencies in Manila
Excellent private hospitals in the metro, a national scheme that covers a fraction of the bill, and insurance that does the real work.
The thing people get wrongPhilHealth is a contribution toward a hospital bill, not cover for it. Private insurance or an employer HMO plan is what actually stands between you and the cost of a serious admission at a Metro Manila private hospital.
Private hospitals
St. Luke's Medical Center in BGC and Quezon City, Makati Medical Center, The Medical City in Pasig and Asian Hospital in Alabang are the institutions foreign residents generally use. Standards at the top are genuinely high, staff speak English throughout, and specialist appointments are direct. Costs are low relative to the West and high relative to local incomes.

- Ask for an estimate before a procedure — hospitals will provide one.
PhilHealth and HMOs
PhilHealth membership is mandatory for employees and contributes a case-rate amount toward covered treatments; the balance is yours. The Philippine solution to that gap is the HMO — a prepaid plan, usually provided by employers, giving access to a network of accredited hospitals and clinics with limits per year. Check the annual limit and the network before assuming you are covered.

- Pre-existing conditions are commonly excluded from HMO cover.
International insurance
For anything beyond an HMO's limit, or for treatment outside the network, international private cover is what fills the gap. Residents planning complex procedures sometimes travel to Singapore or Bangkok, so the evacuation clause matters here as in Jakarta. Check inpatient limits against the real cost of a week at St. Luke's rather than a notional figure.

- Medical evacuation cover, and who triggers it.
- Buy before arrival where possible; exclusions for diagnosed conditions are standard.
Practicalities
911 is the national emergency number. Pharmacies — Mercury Drug above all — are everywhere and dispense a wide range. Dengue is present year-round and rises in the rainy season, and the typhoon season shapes several months of the year: flooding, power interruptions and closures are a normal part of planning rather than an emergency.

- Tap water is generally not drunk; bottled or filtered is the norm.
